Registration

IHS, Tribal, and Urban Nurses are encouraged to attend the NANAINA Summit XII to be held at the:

Registration starts September 8, 2006 and will be accepted on-line, fax, regular mail, and on-site. Download form.

The following registration fees apply:

	Non-member $310.00
	Member $260.00
	Student $60.00

Please note that banquet tickets are included in the registration fee. Extra meal tickets will be available for $60.00 ($40.00 for students).

A pre-conference entitled “Recognizing Toxic Exposure and Contaminants in Patients” will be held November 12th from 10am-4pm presented by the Agency for Toxic Substances and Disease Registry (ATSDR), a Tribal Environmental Health Education Program. Pre-conference is limited to 75 participants. Pre-conference registration fee is $75.00 which includes lunch and materials.

Mention you are a participant of the “NANAINA Summit XII” to secure the special group rate of $165.00 + tax (single/double) per night. The deadline for making room reservations is October 13, 2006. Hotel reservations will be taken on a first-come, first-served basis. Late reservations will be accepted on a space available basis and a higher rate may apply. Please visit the hotel website www.missionpalms.com for more information regarding hotel amenities.

The hotel provides transportation to/from Sky Harbor International airport every ½ hour from 6:00 am – 11:00 pm daily. Courtesy phones are located in the baggage claim area.
